Canterbury CLUB Letter of Intent Form (D1-C1)
Complete this Form to declare your Intent to Test in an upcoming certification testing. By completing this form you are making a firm commitment to attend. Due 4 weeks before the testing date SPRING AND FALL. See Calendar for specific deadlines each year.
Ready to Ride? Submit Coggins & Vaccinations Record
Vaccinations and Coggins Google Form--to verify current Coggins test and Vaccinations. To be submitted by the deadline each year.
Northern Lakes REGIONAL (C2) & NATIONAL Declaration Letter for National Testing (C3/HB and above)
Letter of Intent to Test Form. Due January 1st each year. The letter is non-binding in January but allows the RIC's to plan for testing. There will be another deadline TBD for a final commitment and deposit. Submit to the Regional Supervisor and the Regional Instruction Coordinator via email or snail mail.

Expense Reimbursement Form
Canterbury Expense Reimbursement Form. Please verify with the DC that your expenditures will be covered by the club before you incur expenses. Print or copy this form, complete and submit it with receipts to the Club Treasurer.
